I know there is one in the tab bar but the way I setup my Firefox, it's useless... I need the old button that was customizable.

I always used the "New tab" button in my old version of Firefox. Now with version 4, it's not available anymore

You can drag that New Tab button from the Tab Bar on another toolbar.

Open the Customize window via "View > Toolbars > Customize" or via "Firefox > Options > Toolbar Layout"

The "New Tab" + button may be found on the far right end of tab bar.

Not removed per say.

I have a "List all tabs" on the far right but no "New Tab" button

The default position of the New Tab button is a "+" that appears next to the open tabs and not at the far right, unless you have opened enough tabs to make the scroll buttons appear.

If it is not appearing there then try to reset the toolbar customization.
