2FA QR code, how to generate
I've toggled two-step authentication off/on multiple times, get new set of recovery codes, but I never get the QR coded, how do I generate this, please advise...thanks!

Are you using Sync when doing this? I would say for the trouble it has caused me I don't use it-but make a password uniquely so that I can remember it without having to write it down. To me using 2FA is asking for trouble should you get locked out or password changed because something you did on sync on another computer affects others connected to sync option.
Did you scan the provided QR code with an authenticator app (e.g. Google Authenticator) on the mobile device ?
That's the problem, I don't get the QR code, how do I generate it ?
You should automatically get a pop-up with the QR code once you enable 2FA.
If you use extensions ("3-bar" menu button or Tools -> Add-ons -> Extensions) that can block content (Adblock Plus, NoScript, DuckDuckGo PE, Disconnect, Ghostery, Privacy Badger, uBlock Origin) always make sure such extensions do not block content.
