Why do I find duplicate identical passwords and usernames saved for the same website

When I view list of passwords and usernames I find "double entries" meaning the same username and password for the same site stored twice. Maybe this is because of the "refresh" (should be called deliberate crash) I did (never again) that resulted in an hour long unscheduled interruption while I scrambled to find the solution to restore settings?
