login boxes are missing
when trying to use a website that needs an ID and password, neither box is showing and cant click into the area to enter login id and password

It looks that the login input fields are added via JavaScript and the input fields are a Flash object.
So if you do not see them then check that you aren't blocking JavaScript and that Flash is working properly.
Start Firefox in Safe Mode to check if one of the extensions (Firefox/Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox/Tools > Add-ons > Appearance).
- Do not click the Reset button on the Safe mode start window or otherwise make changes.

And the form is in an iframe hosted on a different server. Very strange.
If the Flash plugin is disabled then I get normal input fields with autocomplete=off, so the script is able to workaround that.
