I use Firefox covered by McAfee on my old Windows 7 computer. I took what looked like a genuine survey and got this response see screenshots. Has Firefox been hijacked?

Believe McAfee. This was likely from malvertising Ads as this is not from Mozilla or the Firefox web browser.

Do not give them any personal information or send them any money by any means.

Variations of this scam that you have won a expensive Samsung or iPhone or a $1000 Amazon gift card has been posted about on forum for a few or so years now. It comes by way of a malvertising Ad and they hope to trick people into giving out personal details like name, address, phone#, email and credit card to pay the so called extremely cheap shipping.

Mozilla has done surveys over the years by way of http://qsurvey.mozilla.com which is surveygizmo.com however no prizes are given.

Oh and If Mozilla (the company that makes Firefox) was legitimately giving out such prizes to random people it would likely be done on a *.mozilla.org page and not some random weird url site.

Those are not from Mozilla. Delete them.

I think your browser is probably fine but you found a deceptive ad designed to capture your personal data by abusing a trusted brand. Not the last time you'll see something like that, I'm afraid.

If you downloaded or installed anything, take a look at these articles:

If you let the site connect to any of your accounts (e.g., Google, Facebook, etc.), go to the account and remove the authorization.
