I get duplicates in my device list and the new device gets the default name instead of the one I entered. Can it be fixed?

I regularly have to remove a duplicate entry for my iOS device because for some reason a new device is created replacing the older one. This causes me to someimes lose tabs that I send to the device because it got replaced for some reason.
