How do I give a toolbar item a new name?

Some time ago, IE8 in my cross-browser setup was unable to access an item on my toolbar and gave it a new name saying so. How can I change that name to something more reasonable, without changing the URL (which is still correct)? I've tried renaming it under IE8, but that change never reached Firefox.

The troubleshooting information below is probably actually from a more recent problem, already reported separately.
