Since v6 I have two title bars on the firefox window

This is like FF is opening inside another window. The top bar says "Mozilla Firefox", and inside that window another window with the new orange button in its titlebar. However, these are not real separate windows as clicking any of the resize icons in the top right of either bar causes both to be resized together, as one. I've tried switching off all the add-ons.
