Whenever I open Firefox, I find all my passwords have gone and I have to re-import the csv file. Why is this happening?

We have been unable to find an explanation of what components of the password file data, like created & modified date, can be included nor any formatting requirements. We've read that only the url, username & password are necessary... does that imply that the date/time fields would not be included if provided when imported?
Following the format of an Exported CSV password file seems reasonable... but risky as it could corrupt the existing file?

A few weeks ago, I started to get repeated messages from Firefox asking me to make Firefox my default browser. Firefox has been my default browser for years, and I have never made anything else my default browser. Anyway, I followed the prompts and re-selected Firefox as my default browser. A little while would go by (sometimes as little as a minute) and I would again get the prompts to select Firefox as my default browser. This append 20 or 30 times. And, every time it happened, all of my saved passwords disappeared, but would reappear when I re-selected Firefox. In the last week, this situation has gotten a lot worse. Now, the saved passwords don't come back. And, the prompt you usually get when entering passwords to a new site offering to save the password doesn't appear any more. So, the only way I can rebuild my saved passwords is to enter them manually, which I am not going to do because it would take a couple of hours and then I'm pretty sure they would all disappear again. Also, I like to go to my homepage when I open a new tab. Can't do that anymore. When I open a new tab, I get a new private browsing window. I have checked all my settings a dozen times, and they are correct. I have combed your support sites and there is nothing there to help me. I don't want to dump Firefox, but I cannot continue with it behaving as it does now. Is there anyone anywhere in your organization who can help me? Thank you.

I've included a screenshot of what I get when clikcing on the Password tab. The information displayed in this wcreen also does not work. Clicking on Sync, does just nothing, just showing Sync in grey... Clicking on Login above just show this screen screen again. Useless.

I use LastPass and I cannot open any login information from the vault. I click on "Launch" (picture uploaded) and nothing happens. If I have a site open I can auto-fill the Username/Password. Some of the sites are tailored to a specific page - I rely on this. It works in Edge and Chrome. It did work before today in Firefox (can't say when specifically). Firefox password management disabled.

I want to erase the username and password for my Amazon account. I prefer to sign in manually. I turned off auto fill in settings, but when I want to shop on Amazon and attempt to sign in, Mozilla no longer autofills but it does provide my username which is my e-mail account. I want the popup window to be blank for my username and my password. The only way I can imagine is to stop Mozilla from "memorizing" this information. Is it possible for me to do that?
