i recently upgraded to version 22.0, but now I get a Javascript error box whenever go to Facebook. I disabled all my add-ons and re-enabled them, one-by-one...so now i know exactly which add-on is having issues.

how and where do i submit a request to get it fixed?

I would say just go to the addons homepage and file a bug/report the problem, e.g. on their respective forums or support channels. You can go directly to the addons sites from your Firefox:

Addons -> choose your addon and then go to homepage.
