setting privacy.firstparty.isolate=true (via about:config) seems to cause Firefox (at least 69.0b3 and tor browser (Firefox 60.8.0esr)) to ignore cookies exceptions (about:preferences#privacy -> manage permissions)

even cookies which are already saved seem not to be used
