Is this from an add-on?

If the add-on has its own preferences dialog, check it to see if there is a setting for displaying the button.

You may also be able to drag and drop the toolbar button from the Customize toolbar window onto the toolbar. For details on how to do that see how to customize the toolbar. 

If the above does not help you can try re-installing the add-on.
