What is the red X before a domain in the password manager ?

I see a red X before a domain in manage passwords. See the attached picture. What does it mean? How to correct that ?

I cannot get into my accounts because I cannot receive a text with a code. My phone is held captive.

Please help me sign in to my hotmail (live) outlook account.
