The "add tab" plus sign is greyed out and ctrl T does not work

I am unable to create a new tab, either by clicking on the "plus" sign [which is greyed out] or by chosing "file, new tab" or by using "ctrl T"

That is a problem with the Ask.com toolbar (Tools > Add-ons > Extensions)
