Exceptions to pop up blockers stop working in the middle of using a web application. Why?

Web applications requires pop ups, so the site is setup as an exception. Works fine for a while, and then all of a suddent Firefox states that pop ups are not allowed. I go into the Options, and it's still listed as an exception.
