I have no address bar. I can't input any url. How do I make it visible?
The Menu bar is CHECKED The Navigation toolbar is CHECKED The Bookmarks toolbar is CHECKED The Add-On selection is CHECKED

Try the following:
- Right click on an empty area in the tab bar
- Select Customize...
- Drag an drop the address/location bar onto an available spot on your toolbar
- Press Ok

The are 16 option to customize my toolbar, but never was there an address bar to select. I still unable to fix this. I have a co-worker with Mozilla and he has it working correctly so I know how it should look. I've attached some photos to illustrate my problem.

Maybe your localstore.rdf file is corrupted. See this page:
I removed the localstore.rdf and had to customize my toolbars again but alas, there still is no address bar to insert into my existing toolbar.

The reset worked!!! I'm back in business. Thanks everyone who gave great troubleshooting advise!

Do you see the location bar in Customize mode?
Are you using a custom theme that might be causing this issue?
Start Firefox in Safe Mode to check if one of the extensions (Firefox/Firefox/Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox/Firefox/Tools > Add-ons > Appearance).
- Do NOT click the Reset button on the Safe Mode start window.
