How to remove an item from the bookmark toolbar?

Somehow, while bookmarking a website my husband managed to place the bookmark directly into the Bookmark Toolbar. I have not been able to remove it using either the Organize Bookmarks or Customize Toolbar features. How do I delete this from the toolbar?
