Can't remove some panes from Top Sites?

I like having the Top Site links available, however I sometimes find that no matter what I do some panes refuse to go away.

I ended up clearing history, cookies, cache, etc to get a blank page and start fresh. However now after one last visit I have Facebook on the page, I site which I deleted everything and will no longer use, and the link/pane/thumbnail cannot be removed no matter how many times I try "dismiss" or "delete".

That are tiles that are automatically added when there is no sufficient history content available.
