[Fx 40] default permissions need update for "Receive Push Notifications".

    Thanks. Screenshot showing "Receive Push Notifications" (in case it's needed) in a default Firefox 40 profile on Windows 7: https://support.mozilla.org/en-US/gallery/image/23158 fx40aboutpermissions

    I'll add a "Needs Change" entry.

    I made a new revision that's pending review, to add "Receive Push Notifications" {for fx40}.

    I also removed old content that was {for not fx29} or below.
