Firefox 12 or Firefox 21??
Version 12 is not secure; Mozilla discloses security flaws after each new release. Is something holding you back from upgrading to Firefox 21? Please let us know so we can suggest solutions or workarounds.
If Help > About Firefox shows Firefox 21.0, you may need to clear the preference that is misreporting your version number. See: Websites say that Firefox is outdated or incompatible even though it's the latest version.

maybe the files in your profile folder that store the information about your extensions might have become corrupted. please go to firefox > help > troubleshooting information > profile - show folder... then windows explorer should open up your profile folder. in there look for the files named extensions.ini, extensions.sqlite, extensions.sqlite-journal (and in some cases also extensions.txt, extensions.rdf). delete or rename those files and quit firefox, they will be regenerated the next time you launch firefox.

You can try to rename or delete the prefs.js file and possible numbered prefs-##.js files and a possible user.js file to reset all prefs to the default value.
I discovered after applying this fix that the issue was due to McAfee's plugin was preventing the install of Tamper. Corporate Policy stuff. Shame it claims to be incompatible instead of saying it was blocked.
A lot of extensions have a maxVersion specified in its install.rdf file that is much lower than the current Firefox version.
The extension file doesn't get updated with each Firefox release, but only when a new version of the extension is released and instead its updates.rdf (or updates.xml) file is updated to confirm that the extension is compatible.
This makes it necessary for Firefox to contact the server and get the current compatibility data that confirms that the extension is compatible.
