Edit: If you are resuming your previous session, then I believe cookies saved in the sessionstore file would be restored so that you can pick up where you left off.

They still appear. They are packed up somewhere and Firefox loyally unpacks them. I am sure that somewhere I clicked OK, but I did so without expecting this.

I Googled rotator.adjuggler.com(note two g's) and it looks as if you have spyware. As a first step you should look through those entries. Also download free Malwarebytes' Anti-Malware and run that.

I use CCleaner to clean my computer, including the cookies I do not want. Recently I noticed that as soon as I open my FireFox 13.0 CCleaner instantly showed 304 cookies had been added. I then deleted my home page, put a blank page as my home page, cleared my history, shut down Firefox, and then ran CCleaner to delete all of the cookies. I made sure all the cookies were gone and then started FireFox again. As soon as it opened, BAM!!!! 304 cookies were installed, many of them for ads!!!!. WHY does Firefox install ANY cookies on opening and how do I prevent this from happening?

out of those at least trackerblock is setting opt-out cookies for advertising networks, so you could consider that intended behaviour.

Can cookies be reconstituted from ETags? I thought that those were being used like super-cookies. I have blocked ETags using Modify Headers, just for good measure, and am no longer getting any of them.

Okay, it does seem that the list in opt-out area of the TrackerBlock is the same as the list. They are all checked. I guess I should uncheck them and see what happens.

Why not check a few of them and then look at what the cookies actually contain. You can view your cookies using Tools > Page Info > Security > View Cookies. Clear the search box at the top so you can see all of them. When you highlight one for a site you opted out of, what does the cookie look like?

Those cookies are maintained and protected by the extension and won't behave like regular extensions because they are meant for special opt-out purposes. If you do not want to see them then you need to disable (remove) the extension.

And if I keep them, they are supposed to Opt-out of tracking me, right?
