It's impossible to dismiss birthday reminders from NextCloud calendars. The calendar in question has been added as a normal CalDAV calendar. The only way to fix this seems to be to completely erase Thunderbird's profile.

If you right-click the calendar in the calendar pane, Properties, is 'Show Reminders' unchecked?
