All browser chrome missing - no header at all

When I move Firefox to my second monitor, all the browser chrome disappears. Here's a screencap: http://screencast.com/t/FkZfSpO1

Only happens with Firefox. All other browsers and programs fine. As you see from screencap, there are no menu options exposed to allow resetting anything. I have uninstalled and re-installed, but because it was last on the 2nd monitor it re-installs there and still no menu.

Could you test in Firefox's Safe Mode? In Safe Mode, Firefox temporarily deactivates extensions, hardware acceleration, and some other advanced features to help you assess whether these are causing the problem.

If Firefox is not running: Hold down the Shift key when starting Firefox.

If Firefox is running: You can restart Firefox in Safe Mode using either:

  • "3-bar" menu button > "?" button > Restart with Add-ons Disabled
  • Help menu > Restart with Add-ons Disabled To do this "blind", press and release these keys in sequence: Alt H R

and OK the restart.

Both scenarios: A small dialog should appear. Click "Start in Safe Mode" (not Refresh).
